‘국제 간호사의 날’을 맞아, 세계의 수백 만 명의 간호사들은 정부가 진정한 연대를 표시하고 보편적 공공보건의료체계의 확고한 구축을 약속할 것을 요구한다

5월 12일(국제 간호사의 날)을 앞두고, 국제공공노련(PSI)은 노동조합과 시민사회가 우리의 선언에 서명하고 보편적 공공보건의료의 확고한 구축을 요구하는데 함께 나설 것을 요구한다

위기로부터 벗어나 사회를 재구축할 때, 우리는 사회가 조직되는 방식을 변혁해야 한다. 우리는 '돌봄'의 역량을 중심으로 우리 사회를 조직해야 한다. 우리 모두는 ‘우리는 하나다.’라는 노동조합의 격언을 깨달아야 한다. 우리의 보건의료 시스템이 모든 사람을 돌볼 수 없다면, 우리 모두가 위험한 상태에 있는 것이다.

오늘날 정부가 간호사들에게 존중을 표시할 때, 우리는 정부가 다음의 근본 원칙을 인식할 것을 요구한다.

  • 정부의 가장 중요한 목적은 모든 사람이 보살핌을 받을 수 있도록 사회를 조직하는 것이다.

  • 우리는 질병으로부터 이윤을 뽑아 내는 비뚤어진 관행을 더 이상 용납할 수 없다.

  • 보건의료 서비스가 지불 능력에 의존하게 해서는 안 된다.

  • 무역은 양질의 공공보건의료를 제한하는 것이 아니라 양질의 공공보건의료를 제공하는 국가의 역량을 강화해야 한다.

아울러 우리는 다음을 요구한다.

  • 정부는 간호사 및 간호사 노동조합과 협력하여 공공보건의료 재건계획을 만들어라.

  • 정부는 생명을 구하는 약품, 장비, 진단법, 백신과 같은 의약품을 적시에 적정 가격으로 공급하는 것을 방해하며 현재의 위기에 대처하는데 필요한 모든 조치를 취할 수 있는 정부의 역량을 저해하는 모든 장애물(지적재산권 규칙, 기존 무역협정과 규칙 포함)을 제거하라.

  • 모든 정부는 코로나19와 관련된 모든 연구, 데이터, 기술, 치료제와 백신을 위한 ‘지구적 코로나19 공유재’를 독점 판매권이 없는 공유된 지구적 자원으로 개발하자는 코스타리카 정부의 제안을 지지하라.

  • '세계은행'은 공공보건의료를 위한 자금을 민간보건의료산업에 제공하는 것을 중단하고, '국제금융공사'는 공공민간파트너십이라는 잘못된 모델을 포함하는 보건의료민영화 촉진을 중단하라.

  • '국제통화기금'은 정부에게 공공지출을 줄이고 공공부문임금을 삭감하라는 지침을 내리는 것을 중단하라.

  • 모든 노동조합과 시민사회조직은 보편적 공공보건의료의 확고한 구축을 요구하는데 함께 나서자.

우리는 과거로 되돌아갈 수 없다!

유엔세계보건총회는 2020년을 ‘국제간호사·조산사의 해’로 선언함으로써 간호사외 조산사의 공헌을 기리기로 결의하였다. 올해 5월 12일(‘국제 간호사의 날’)은 플로렌스 나이팅게일 탄생 후 200년이 되는 해이며, 공공보건의료에서 간호의 역할이 공식적으로 인정된 지 200년이 되는 해이다.

코로나19 유행병 한가운데에서 맞는 올해는 다른 어느 때보다 더, 간호사들이 ‘무명의 영웅’이 아니라 모든 사회의 존경을 마땅히 받고 제대로 된 임금과 보호를 받아야 하는 전문가로서 인정받고 찬양 받아야 한다.

코로나19 위기는 공공보건의료 시스템에 대한 여러 해에 걸친 자금지원 부족과 신자유주의라는 이데올로기 바이러스에 의해 더 악화되었다.

아래에 서명한, 간호사와 보건의료노동자를 대표하는 노동조합들은 2020년의 ‘국제 간호사의 날’이 정부가 신자유주의와 긴축이라는 근본적으로 위험한 실패한 이데올로기를 벗어버리고 그 대신 보편적 공공보건의료에 대한 대규모 투자를 통하여 경제를 재건한다는 약속을 하는 날로 기념되기를 희망한다.

코로나19 유행병은 공공보건의료 노동자가 우리의 생존에 얼마나 필수적인가를 증명하였다. 그러나 간호사들은 위험하게 과로에 시달리고 있고 적절한 보호장비를 지급받지 못하고 있으며, 트라우마와 스트레스로 고통 받고 공격과 혹사에 노출되어 있으며, 심지어 자신의 집에서 쫓겨나고 있다.

국제공공노련(PSI)이 실시한 보건의료노동조합에 대한 조사에서, 56.5%의 응답자들은 코로나19 유행병에도 불구하고 노동자들이 적합한 개인보호장비를 지급받지 못하였다고 지적하였다. 우리는 얼마나 많은 보건의료노동자가 감염되었는지 그 정확한 숫자를 알지 못하지만, 수백 명의 보건의료노동자가 이미 죽었다는 것을 알고 있다.

코로나19 위기는 공공보건의료 시스템에 대한 여러 해에 걸친 자금지원 부족과 신자유주의라는 이데올로기 바이러스에 의해 더 악화되었다.

보건의료산업은 세계적으로 대규모 성장산업의 하나이지만, 필수적 공공보건의료서비스에 대한 공공 자금지원은 필요에 못 미치고 있다. 너무 많은 국가에서, 보건의료에 대한 공공적 자금지원은 의료산업이 부풀린 비용에 의해 잠식당하고 있다. 너무 많은 간호사와 기타 보건의료노동자가 단기 계약이나 불안정 계약으로 고용되어 있다. 이제 열악한 임금과 노동조건이 공공보건의료를 위험에 빠뜨린다는 것을 인식할 때이다.

보건의료시스템에 대하여 적절한 자금 지원이 이루어졌고, 정부가 간호사/환자의 비율을 법제화하였으며, 정부가 생명을 구하는 개인보호장비, 의료장비, 의료연구, 백신과 치료제의 생산역량을 확보하고 있었다면, 보건의료시스템은 코로나19 유행병에 그렇게 무방비 상태에 놓이지는 않았을 것이다.

세계보건기구는 최근에 발간한 '세계간호노동력현황보고서'에서, 코로나19가 발생하기 이전에도 세계는 최소한의 보건의료 필요를 충족하기 위해 6백만 명의 간호사가 추가로 필요했다고 지적하고 있다. 정부는 간호 교육과 훈련을 위한 자금을 확대해야 하며, 자신의 공공보건의료 필요에 대처하기 위해 애쓰고 있는 국가들로부터 간호사를 수입하는데 의존해서는 안 된다. 간호사를 확보하고 유지하는 가장 좋은 방법은 간호사 노동의 사회적 가치를 인정하고 적절한 임금을 지급하며 적절한 인력수준을 확보하는 것이다.

세계의 공공보건의료와 개인의 건강권은 공공보건의료의 모니터링, 보건의료연구, 건강 진단법과 데이터부터 3차 의료와 의약품 공급에 이르기까지 공공보건의료서비스의 모든 요소들이 공공시스템을 통해서 소유되고 관리되며 인력 충원하고 전달되어야 비로소 성취될 수 있다.

공공보건의료를 위한 자금을 다른 공공서비스로부터 뺏어와서는 안 된다. 개발도상국들에게 추가적인 부채를 강요해서도 안 된다. 그 대신 우리는 현재 30조 달러를 조세회피처에 감출 수 있게 해주는, 개별국가와 세계의 조세금융규칙의 개혁을 요구한다. 다국적기업과 억만 장자들이 조세회피조치를 이용할 때, 그들은 우리의 공공보건의료 시스템을 의도적으로 훼손하고 있으며, 간호사, 보건의료노동자, 그리고 제대로 작동하는 건강한 사회에 필요한 공공서비스를 제공하는 사람들의 임금에 기여하기를 거부하고 있는 것이다.

위기로부터 벗어나 사회를 재구축할 때, 우리는 사회가 조직되는 방식을 변혁해야 한다. 우리는 돌봄(care)의 역량을 중심으로 우리 사회를 조직해야 한다. 이윤과 소비를 뽑아 내고 늘리기보다는 모든 사람의 돌봄과 복지가 사회의 중심적 조직원칙일 때 모든 사회가 건강하게 발전할 것이다. 우리 모두는 ‘우리는 하나다.’라는 노동조합의 격언을 깨달아야 한다. 우리의 보건의료 시스템이 모든 사람을 돌볼 수 없다면, 우리 모두가 위험한 상태에 있는 것이다.
